Dr.Bill will refer physicians to Madaisky Pollock LLP for legal services related to the incorporation of their medical practice. Madaisky Pollock LLP will provide a full (100%) discount on its customary flat legal fee up to $1,500 Offer open to any medical residents or those within 5 years of completing their medical residency, for incorporating once they are eligible to and incorporate pursuant to the bylaws of the College of Physicians and Surgeons of B.C.

As part of this offer, Madaisky Pollock LLP will provide the following legal services related to the incorporation of the medical practice:

• Preparation of application to the College of Physicians & Surgeons for consent
• Letter to the College
• Attend to name reservation with the Registrar of Companies
• Arrange for filing with the Registrar of Companies
• Preparation of all post incorporation documents, including consent to act as director and officer, share subscription, directors and shareholders resolutions
• Notice of issuance of shares, employment documents, registered and records office agreement
• Attending to all necessary filings; and to all other telephone calls, conferences, correspondence and matters necessary and incidental to the foregoing

This offer is limited to legal fees only; physicians are required to pay all disbursements, taxes and other associated costs.
